Providing complete care to frail or elderly patients can be challenging because individuals may have multiple health problems along with limited resources and support. To help the family physician obtain a complete picture of patients’ health, the CWC PCN has a Comprehensive Geriatric Assessment (GAT) team to conduct a thorough evaluation of geriatric patients.
The team includes a social worker, a pharmacist, a geriatric psychiatrist and geriatric nurse specialists who assess patients for the following concerns:
• Safety (e.g., home hazards, falls, medications)
• Functional capacity (daily living, driving, transportation)
• Chronic disease management
• Mental health (cognitive function, dementia, depression)
• Psychosocial issues (family support, social needs).
Services may be delivered in a medical or community clinic, in the patient’s home, or by telephone.
