Geriatric Assessment and Support Clinic (GAS) 

The Geriatric Assessment and Support (GAS) clinic accepts frail, elderly or geriatric patients for up to five visits to assess for care needs. The clinic’s multidisciplinary team includes nurses, family physicians, a pharmacist and a social worker.

The clinic is specially set up to conduct comprehensive assessments and care plans for patients. The clinic can help patients access medical and community services; provide support for personal needs; and identify care gaps that need to be addressed. Once the evaluation and care plan are developed, they are provided to the family physician to manage care in the long-term.
