The Behavioural Health Consultant (BHC) team of the Calgary West Central PCN can help by working alongside physicians to give patients the extra support they need. The BHCs can help identify barriers to change, offer motivational support, or direct patients to services and supports available in the community. It’s all part of a program that gives patients a little extra time with the many health professionals at the CWC PCN.
