Patients of the CWC PCN can count on a team of nurses to provide care that goes well beyond the services available through physicians alone. Our nursing team is set up to spend extra time with patients in important areas such as education and self-management, follow-up and scheduled testing.
Nurses also play an important role in arranging for routine screening and monitoring (e.g., mammograms, cholesterol testing, blood pressure testing) and alerting the family physician to new concerns. Best of all, our nurses are easier to reach by phone, email and in-person visits, meaning patients can ask questions and get advice in between visits with their family physician. It’s a win-win for patients and physicians alike.
